INFORMATION AND CONSENT for the processing of personal data
BASKETBALL CLUB 'SOFIA EAGLES', UIC: 207306806, with its registered office and management address at Sofia Region, Sofia Municipality, Sofia 1766, Vitosha District, Multifunctional Complex Garitage Park, Donka Ushlinova Street No. 2, Block Office Building 3, Floor 2, (the 'Club'), wishes to use the following information about you, constituting 'personal data' within the meaning of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (any information relating to an identified or identifiable natural person ('data subject'); an identifiable natural person is one who can be identified, directly or indirectly, in particular by reference to an identifier such as a name, identification number, location data, online identifier, or to one or more factors specific to the physical, physiological, genetic, mental, economic, cultural, or social identity of that natural person):
- Full name, Personal Identification Number (EGN), Place and date of birth, School, Phone number, Height, Weight, Information on chronic illnesses;
- Full name, phone number, and email address of the parents
The information you have provided above will be used for:
- For the administration of the club’s activities, conducting competitive activities, organizing sports camps, tournaments, and excursions
The information you provide will be used in the following processing activities:
- Storage and transfer to another administrator for the purposes of maintaining the club’s regular accounting
- Transfer for the conduct and administration of competitive activities at the membership level within basketball club associations in Bulgaria;
- Transfer for the organization and conduct of sports camps and trips
The information you provide will be used for a specific purpose and, depending on that purpose, will be retained for a specified period, which is
- For the duration of the child's membership in the Club
After the purpose for which your personal data was provided has been fulfilled, or upon the lapse of the legal obligation and/or our legitimate interest, we will securely delete or destroy your personal data.
In case that you do not provide the required information, including the necessary personal data, BASKETBALL CLUB 'SOFIA EAGLES' will not be able to fulfill its legal and contractual obligations arising from its status as a sports club, including, but not limited to, enrolling your child in the club’s competitive activities and granting membership
In cases where the processing of your personal data is based on your consent, the consent you have given for the processing of personal data may be withdrawn at any time. Your consent should be withdrawn through an explicit written statement submitted to the Company at its registered office and management address or via email: [email protected]
BASKETBALL CLUB 'SOFIA EAGLES' undertakes to guarantee you the right to request access to, correction or deletion of personal data, or the restriction of processing of personal data related to the data subject, as well as the right to object to processing and the right to data portability. Furthermore, when the processing is based on Article 6(1)(a) or Article 9(2)(a) of the Regulation, you are granted the right to withdraw your consent at any time, without affecting the lawfulness of the processing based on consent before its withdrawal. BASKETBALL CLUB 'SOFIA EAGLES' also informs you that you have the right to lodge a complaint with a supervisory authority
By signing this document, I declare and confirm that I am familiar with the terms regarding the processing of personal data by BASKETBALL CLUB 'SOFIA EAGLES' and I do / do not give my consent for the provision of the requested information to BASKETBALL CLUB 'SOFIA EAGLES' as provided in the information form I have completed for the Club
